In order for results to be recognized in court, the identity of the parties tested must be confirmed and the collection process must be witnessed.

Chain-of-Custody Witnessing Instructions:

• The sample collection process must be witnessed by an unrelated, unbiased witness (such as your physician, your lawyer, a notary, your pastor, etc.)
• The witness must verify the identity of the test participants by confirming two pieces of legal identification (e.g. driver's license, passport, etc.)
• The witness must sign the completed Chain-of-Custody Witness Document prodvided in the kit as a witness to the collection.
• The witness must seal the specimens in the container provided in the kit, sign the security seal, and send the samples directly to the laboratory for testing.
